• Camper Application (200kb) - 15 page application campers are required to complete for camp. This is a large file so it may take a minute to download, depending on your connection speed.
• General Volunteer Application (69kb) - application for those who only want to volunteer for one day.
  
• Massage Staff Application (83kb) - to be completed by massage staff volunteers.
  
• First-time Volunteer/Counselor Application (132kb) - this is for individuals who have never volunteered or served as counselors for Camp Dreamcatcher.
• Returning Volunteer/Counselor Application (113kb)- if you're a returning volunteer or counselor, you'll only need to complete this condensed application.
• Counselor-in-Training Application (CIT) (187kb)- for future counselors who are sixteen (16) years old.
• Junior Counselor Application (188kb) - for those who have participated in the Counselor-in-Training (CIT) program and are ready to take the next step as counselors and are seventeen (17) years old.
• Healthcare Volunteer Application (75kb) - a medical summary and application to be completed by new health care volunteers.
• Returning Healthcare Volunteer Information Sheet (250kb) - a medical summary to be completed by returning health care volunteers.
